Targeted Liposuction
Most excess fat deposition in the body can be controlled through diet and exercise.
However, there are stubborn fats in the body which, due to their location and inherent properties, refuse to disappear no matter how hard we try. This commonly includes fat at the neck and chin (‘double chin’), the upper outer breast and axilla region (‘bra fat’ or ‘armpit fat’), and the upper arms.
ONFA (Once ‘N’ For All) Lipo is the use of targeted liposuction to permanently* remove these stubborn areas of fat.
*Results may vary from individual to individual.
Ideal Candidates
The ideal candidate for ONFA Lipo is an individual who has been persistent in a weight loss or weight maintenance regimen, including proper diet and exercise, and is of a normal body mass index (BMI), and are unable to get rid of stubborn bulges in certain parts of the body, despite their efforts. As mentioned previously, these commonly include the double chin, bra fat, and upper arm fat.
Individuals who are overweight or obese and who have not tried to lose weight on their own will find that fats removed from liposuction tend to reaccumulate if no effort is made to maintain the weight loss.
The amount of fat removed in ONFA liposuction is relatively small, and the procedure can be performed comfortably in our day surgery unit under an hour. It is commonly performed under local anaesthesia with a small amount of sedation to keep you comfortable during the procedure.
You will be monitored closely in the operating theatre. Once sedated, small stab incisions are made based on pre-operative planning to allow access to the targeted areas. This is then followed by liposuction, ensuring adequate volume removal and symmetry. In cases where tightening of the skin is desired, VASER liposuction technology can be added to help with post-operative skin retraction.
ONFA liposuction can be performed concurrently with other treatments to enhance overall results. These include minimally invasive procedures such as fillers as well as surgical procedures such as facelifting. Your experienced plastic surgeon will perform a thorough assessment and discuss the best treatment options for you.
UNDERSTANDING PAH
Paradoxical Adipose Hyperplasia (PAH) is a rare side effect that can occur after non-surgical fat-freezing procedures, where fat cells grow instead of shrink. This rare condition leads to firm, noticeable lumps that don’t go away over time. PAH typically requires corrective surgery such as liposuction to reverse the effect
Unlike fat-freezing treatments, ONFA Lipo directly removes fat cells, minimising the risk of complications like PAH and offering a more predictable, targeted solution for stubborn fat.
patient-centric plastic surgery practice
Your doctor will assess the individual’s unique anatomy and desires to recommend the appropriate treatment. Besides ONFA Lipo, treatments may include excision of excess skin or minimally invasive skin-tightening procedures. In addition, VASER liposuction can also be combined with ONFA Lipo to enhance the post-procedure skin retraction and smoothening.

The risk of skin sagging will be dependent on the main component of the area for liposuction. If the area consists mainly of fat with firm skin, the likelihood of skin sagging is low. If the area consists mainly of skin, you may want to discuss other options such as excisional procedures
The areas that ONFA liposuction targets are mainly fats which are refractory to diet and exercise. This fat, once removed, usually stays away. However, if you do not follow a good exercise and diet program after the procedure, there could still be subcutaneous fat which accumulates leading to the appearance of the original fat coming back.
The cost of ONFA Lipo may vary from individual to individual, and begins from $5,000 (all inclusive).
Expected sequelae of the procedure include pain, bruising and swelling. Pain is usually well controlled with oral painkillers, whilst bruising normally dissipates after 2 to 3 weeks. Swelling at the area of liposuction is maximal during the first week after the procedure. After this, the swelling will gradually dissipate, and the final results can be seen at 8 to 12 weeks after the procedure. You will be required to wear a pressure garment for as long as possible in the weeks after the procedure to help with reduction of swelling and skin retraction.
At Polaris Plastic & Reconstructive Surgery, we will assess the individual’s unique anatomy and desires to recommend the appropriate treatment. Besides ONFA Lipo, treatments may include excision of excess skin or minimally invasive skin-tightening procedures. In addition, we can combine VASER liposuction with ONFA Lipo to enhance the post-procedure skin retraction and smoothening.
Potential complications, though rare, can include:
Possible contraindications for ONFA Lipo include multiple uncontrolled medical problems, unstable weight gain or loss, weight gain due to pre-existing medical conditions, previous surgery in affected or targeted area, or suspected/confirmed cancer or on cancer treatment.
