How the Crease Forms and Why It Looks Natural

If you were born without a double eyelid, it is because the levator palpebrae superioris (the muscle that lifts the eyelid) lacks fibrous attachments to the upper lid skin, so nothing folds when your eye opens.

Double eyelid surgery creates that attachment point. When placed at the right height, the fold mirrors a natural crease — deepening when you look up, softening when you look down, sitting in proportion with your eye.

Changes to Eye Shape, Openness, and Proportion

The most visible change is greater eye openness. More of your iris becomes visible, making your eyes appear larger and more defined. The procedure may also improve symmetry if one eye naturally has a faint crease and the other does not. If you have mild ptosis (drooping caused by a weakened levator muscle), ptosis correction may be performed at the same time.

The Results Timeline: What to Expect Week by Week

Recovery is gradual. Knowing what to expect at each phase makes it easier to stay the course.

Immediately After Surgery

In the first 24 to 48 hours, your eyelids will be swollen, and the crease will appear more pronounced than your intended result. This is a normal tissue response, not an indication of how your eyes will look once healed.

Week 1 to 2: Why Results Look Exaggerated

Your crease may look dramatic, and your eyelids may feel tight or heavy. Bruising is common, particularly with the incisional method. What you’re seeing is swollen tissue above the crease;, the fold will look less severe as it settles.

Months 1 to 3: Crease Settles

By the end of the first month, most visible swelling has resolved and the crease begins to settle. If you had the incisional method, some residual swelling can persist for up to three months. Temporary asymmetry between the two eyelids is normal at this stage.

6 Months and Beyond: Final Results

Your double eyelid surgery before and after comparison is most meaningful from around six months, though if you had the incisional method, subtle refinement may continue to 12 months.

  • One side remains noticeably higher, lower, or more defined than the other
  • A suture-method crease begins to fade or disappear
  • The fold looks static or mask-like at rest

Revision Surgery: What It Involves

Reversal is possible in some cases, but not straightforward. With the non-incisional method, sutures can, in principle, be removed early, though this may not restore your original appearance.
